• Account No. 31219713-NADRA - Fee has to be deposited in HBL Rotterdam or else a bank draft can be made in the name of NADRA, Islamabad.
• Copy of the bank deposit slip/receipt should be sent along with the application form.
• Following are to be submitted along with the application (which has to be accurately filled in black ink) three photographs (preferably colored)
• copy of foreign/Dutch passport
• copy of Pakistan passport
• copy of Pakistan NIC/form RG-II (in case of persons under 18)
• Bank deposit slip/bank draft

Application form has to be submitted with the Embassy of Pakistan.
